Young Epilepsy – support for 16 – 25 year olds with epilepsy

Brief description of what services your organisation offers

We provide support for 16-25 year olds that have epilepsy to help them get into training, employment, education or voluntary work. We have a holistic approach and strive to achieve a well-rounded quality of life, sense of purpose and network of friends.

Where does the service take place?

Hampshire including Southampton and Portsmouth

What the organisation provides

Young Epilepsy Transition Support is a free project working with 16-25 year olds, with epilepsy living in Hampshire. Whilst young people with epilepsy have support from their health professionals, we recognise that other areas of their lives may be impacted quite severely, such as housing, independence, developing their personal, social skills and feeling very isolated. We encourage young people to have a sense of purpose, gain a network of friends and to live a well-rounded life. The Transition Support Project works with young people (often 1:1) to support them to identify and overcome issues or barriers to further education, training, employment or any other areas of their lives. The project also works alongside other organisations, to support them by supporting your young people such as at appointments, activities, volunteering and employment.

Who can access the service?

16-25 with epilepsy living in Hampshire.

Expected outcomes for children and young people and their families accessing your service

The outcomes are very individual and would depend on the needs of the young person and family. The project is intended to support young people into education, training, volunteering or employment as well as overcoming any barriers to this or other areas of their lives.

How can this service be accessed?

Please contact us to make a self-referral or to make a referral for a young person and we can send a referral form. Please fill this out as comprehensively as possible, the more information we have the better. We try to see people as soon as we can this is often within a few weeks.
