School Age Immunisation Service
The local NHS School Immunisation team will visit your child’s school at various times throughout the year to deliver the childhood vaccination programme.
Parents/carers will receive information about the vaccines from school when they are due. This will include instructions for accessing the electronic form, along with a unique code. Please complete the form to indicate if you do or do not consent for your child to receive the vaccination.
If your child is registered as Electively Home Educated the School Immunisation team will contact, you by email or post to complete the electronic form. After you have completed the form, you will receive details on booking an appointment within a community clinic, for the vaccination.
If the Immunisation team have not received a completed form for your child or your child misses their scheduled vaccination, they will continue to offer the vaccine until the child leaves school or reaches the age of 16 years.

What support is there for children with additional needs?
We are able to offer children with additional needs extra support to have their immunisations. Some of the ways we can support are as follows:
- Social stories
- Pre visits to see what happens at the session or appointment
- Distraction toys / cold spray
- Longer clinic appointments
- Dedicated vaccination room
- Possible home visit
- Vaccination alongside medical procedures with sedation / anaesthetic
We also operate SEND clinics which are only for children who have additional needs where individual rooms are allocated and appointment times are longer.
If you feel your child would benefit from additional support or you would like to book into one of the SEND clinics, please contact us to discuss this.
You can also read leaflets published by the Government for parents and educational settings about supporting vaccinations for SEND children and young people.
What if I need help?
If you have any questions about the vaccines, completing the electronic form or would like to discuss adjustments/changes required to support your child having their vaccine, please contact the Immunisation Team.
Whilst most children have their vaccinations in school, where this is not possible, we can offer an appointment in a local community clinic. In exceptional circumstances, a home visit may be offered.
