Equipment Service

Brief description of what services your organisation offers

The principal aim of the service is to enable those individuals in need of support, some of whom are vulnerable, to maintain their health, social care and educational needs and well-being in their chosen location for the foreseeable future and to help alleviate risk (e.g. falls, pressure sores etc.), promote independence and facilitate end of life strategies.

Where the service is provided

This service is delivered from Unit 12/13 Railway Triangle, Walton Road, Portsmouth. The service covers the whole of the City of Portsmouth.

What the organisation provides – types of interventions and support

The service has been commissioned to provide a range of standard equipment and can provide bespoke specialist equipment for service users where prescribers consider this is appropriate. Equipment can be provided to support health needs, social care needs or in some cases educational needs.

Who can access the service and how can it be accessed

Service users will be assessed by prescribers who can be based in a range of teams; Children’s Social Care, Children’s Physiotherapists, , District Nurses, Occupational Therapists, Hospital Teams etc.

Quality standards and expected outcomes for children and young people and their families accessing your service

There are a range of quality requirements under the contract for the service that will be monitored on a monthly basis including, prompt delivery/collection, regular maintenance, clear guidance on usage of items etc. The expected outcomes for each individual service user will be set by the prescribers who are separate from the Equipment Service.

Complaints are shared with commissioners on a regular basis and reviewed to ensure issues are resolved.
