Brief description of what services your organisation offers:
Enable Ability's Community Based Support Service provides a service at a time to suit the parents of a child with mild to moderate levels of disability. 
Where the service is provided - please state if City wide or locality based:
The service is for parents of children living within the Portsmouth city boundaries.
What  the organisation provides - types of interventions and support:
The Community Based Support Service is designed to either take place within the family home or within the local community if this is felt to be more appropriate to meeting the family need.
Eligibility Criteria:
The service is for families of targeted children with moderate levels of disability in accordance with the criteria set out be Portsmouth City council.
Quality standards and expected outcomes for children and young people and their families accessing your service:
The family receive breaks at times that best suit them whilst having re-assurance that their special needs child is being supported by a suitable trained staff member.  Outcomes for children are agreed at the time of commencement of the service.

Via the Short Breaks Officer at Portsmouth City Council in the first instance (although initial enquiries can be made direct to Enable Ability as required).
